Tornado Relief for Affected Seniors
Have you been impacted by the May 16 tornado?
LifeWise has been chosen by the city of St. Louis to serve seniors (age 65+) in Disaster Case Management. Survivors may sign up for this program to be connected with resources, services, and support, with the goal of rebuilding your life and becoming self-sufficient again. Request case management services using this online form.
Requirements:
- You live in the City. Check your address to make sure it’s within City limits.
- You are a survivor of the May 16th Tornado with remaining unmet disaster-related needs (such as housing, employment, health, or financial issues not fully covered by insurance or FEMA aid).
If you meet requirements, a trained disaster case manager can provide one-on-one assistance to:
- Assess unmet disaster-related needs.
- Develop a recovery plan.
- Find and apply for resources (FEMA aid, housing, repairs, legal help, mental health care, etc.).
- Coordinate with nonprofits, local agencies, and government programs.
- Navigate available aid programs and rebuilding steps.
