The following resources are available to the community.
Community Care Liaison
The LifeWise StL Community Care Liaison supports adult individuals and families when they initially connect with LifeWise StL. The Community Care Liaison can help address immediate basic needs, and appropriately refer individuals to internal and external programs.
If you are in crisis or just need some support, the Community Care Liaison can help you connect to local resources, connect to programs and services at LifeWise StL, understand eligibility for social services, apply for Food Stamps or Medicaid, and more.
Call 314-329-7147 to schedule an appointment.
